Description
The Dane County Division of Public Health - Public Health Nursing section provides a variety of public
health-related programs and services related to the overall health and well-being of Dane County residents
living outside the City of Madison.

General Eligibility
|
|
Public Health Services are available only to those Dane County residents living outside of the City of Madison
with the following exceptions: the Immunization Program, Seal Dane Program, Tobacco Free Dane County Coalition,
and the Wisconsin Well Woman Program. There is no direct cost for most services. There may be eligibility
requirements for some programs and services.

Services

|
Chronic Disease Prevention - The Dane County Division of Public Health
provides chronic disease prevention information, education, and resources to help Dane County residents make
healthy life choices, reduce the human and economic impact of poor health, reduce the incidence of premature
death and disability, and promote healthy communities.
The recently released 2005
Chronic Disease Health Status Report for Dane County shows that while the county's population is meeting
some health goals, there is much room for improvement in preventing chronic diseases.
Communicable Disease
Control - A team consisting of a Public Health Epidemiologist and Public Health Nurses provide
confidential consultation and follow-up for surveillance and control of specific infectious diseases, including
food- and water-borne, sexually transmitted, vector-borne, and vaccine preventable diseases.
Community Education and Health Promotion - Public Health staff and
community partners collaborate to promote health and prevent communicable and chronic diseases, injuries and
disabilities related to unintentional injuries. Public information and education is provided through
newsletters, press releases, radio and television presentations, media campaigns, exhibits and presentations at
community events, and educational seminars.
Dental Health - The Dental
Health program provides free fluoride supplements, dental health education, community sealant programs, dental
screenings, dental health education materials, and access to care.
Immunizations - The full range
of childhood immunizations, as well as, some adult immunizations are provided at no charge on a regular basis at
Public Health Clinic locations throughout Dane County.
Pregnancy and Child Health - The
Department of Human Services - Public Health Division provides an array of pregnancy and child health services.

Women, Infants, and Children (WIC) Nutrition Program
Public Health Clinics -
Public Health Clinics are held on a regular basis at various community locations in Dane County. Free walk-in
services include: immunizations, blood pressure screening, health information, and referral to other services.
By arrangement (call 608-266-4821), tuberculin skin testing is provided (there is a fee for individuals not
considered to be in high risk categories).
Well Child Clinics - Complete physical exams are provided at Well Child
Clinics for anyone uder the age of 21 who is uninsurable. Included are hearing and vision screening for persons
age 3 years and older and lead testing for persons age 12 months to 6 years. Transportation and interpretive
services will be arranged as needed.

Wisconsin Well
Woman Program (WWWP) - is a statewide program that provides free or low cost health screenings,
diagnostic tests and referrals related to: breast cancer and cervical cancer for eligible women.
To apply, contact Dane County Department of Human Services - Public Health Division at (608) 242-6392. Spanish
speaking individuals may contact the Dane County WI Well Woman program at (608) 242-6235.
Women, Infants, and Children (WIC)
Nutrition Program - is a federal program that provides vouchers for supplemental food and nutrition
information for eligible pregnant women, breastfeeding women, postpartum women, infants, children under the age
of 5, and fathers or foster parents of eligible infants and children.
